Nobody should grade their own care home compliance

Bello Endurance · Elberra Consulting International ·

Bello Endurance is the Managing Partner of Elberra Consulting International. This Consulting Rainmakers episode, hosted by Mikulas Zach, is about one idea, in Bello's words: Nobody should grade their own care home compliance.

Why should care homes get an independent compliance audit?

Bello Endurance: Operators are too familiar and confident in their own systems to judge them objectively. An outside reviewer is unbiased and checks every part of the operation against what the regulator expects.

In this conversation, Bello explains why safeguarding is one of the biggest issues behind providers getting into trouble with the CQC, the core things he tests first when a care provider comes to him, and why every provider needs an unbiased third eye on its own operations.

About Bello Endurance

Bello Endurance is a Chartered Accountant (ACA, ICAEW) whose career runs through PwC, where he was a Financial Services Manager on secondment to Unilever as a Financial Accountant, CroydonPlus Credit Union, where he headed financial control and compliance and later sat on the Supervisory Board, and Community Living London, where he led preparation for CQC audits and inspections as a health and social care compliance consultant. He founded Elberra Consulting after seeing a gap in the UK healthcare sector: a lot of money invested, yet care that did not meet the standard people pay for. Elberra provides CQC consulting, mock inspections and accounting for health and social care providers, and he has supported more than 200 care providers.

Expect to learn why investing in a care home is not the same as knowing how to run one, how recurring training for leaders helps them handle a safeguarding issue and pass on the lessons learned, why it is better to open up about a problem than to hide it, how AI is already taking on documentation, risk assessments and care planning, why the one thing AI cannot replace is your own willingness to keep learning, how Elberra wins clients through articles, Google ads and referrals, and where he wants to take the firm next, and much more…

Mikulas Zach

Today I'm sitting down with Bello Andurens, the managing director at Elbera Consulting. Bello, welcome.

Bello Endurance

Thank you very much, Mike.

From PwC To Founding Elberra

Mikulas Zach

So start me at the beginning. you spent a few years at PWC. what what made you you know then move through roles and actually you know founding Albera consulting? What's the story?

Bello Endurance

thank you very much. I mean, working in consulting has always been my goal, and I was quite privileged to have the opportunity to work with PWEC and some other consulting firm. thinking of starting the barrier was the gap which I noticed in the healthcare sector. I felt that I needed to do more and I also need to provide services to, you know, the users and then the operators. In United Kingdom, for example, you have a whole lot of money invested in the healthcare sector. And for me, when I came in, I noticed there is that gap of compliance. You know, people want to take medications, people want to take they want to they want services, but yet the quality of services they want really is not what they get. that is what I call value for money. And I saw that gap and I felt there is need for us to fill that gap here in the United Kingdom so that The two-way, that is, those who are benefiting from the system and those who are providing for the system would have opportunities to maximize efficiency, maximize return on their money for those who are paying the tax. And so that is how Ebera came in. So Ebera came in because of the pain point we see in the market, in the healthcare sector. And what what we strive to achieve is to ensure that we provide compliance that will make sure that providers meet up to the standard which is required of them. Like I was trying to tell you before the show, for example, we have irreg regulators here in the United Kingdom. they generally call them CQC, right? They regulate the health and sharecare sector. Right? So we w we find on many occasions that some people really do not understand what they are supposed to do. They just want to invest in the health and sharecare sector and yet they are not properly run. And in that situation You are going to have two blind people leading themselves and what will happen, both of them will fall into a dish. And so what we decide to do is to provide the compliance aspect of it. Why operators operate, and then we provide the compliance in line with the standard required of them by CQC. What is my joy or my goal? My goal is to ensure that every healthcare provider meets up to the standard required of them. For example, the CQ CQC will tell you, we want you their call for wanting to make sure that the organization is well led, they are efficient, they are caring, they also have what they call efficiency in terms of running the system. And so what we check in our own end is to ensure that the system, if you have a care home or a supported living, for example, we want to check whether you are running that operation in line with this standard that is set by CQC. And so we test those system. We do the we check whether you are running efficiently, whether you have the right training, whether your management is non-legible to operate in that standard which is required of them. And we want to make sure that the people whom have been trusted entrusted to you to care, you're actually rendering the care that really meet the standard that the government wants, that the people want and the society wants. And that is where a barrel really comes in to play here in the United Kingdom. And from the other side of it, we talk about what is my joy. My joy is to make sure that people who are enjoying healthcare services are happy to ensure, are happy that what they are enjoying, what they are getting, the services they getting is of utmost standard. You understand? So that is my joy. And that is what we bring in. That is the passion that motivates us.

The Most Common Compliance Gaps

Mikulas Zach

Right. so given that you know you've served well over two hundred care providers, you mentioned a gap that you help fix companies in healthcare. what are kind of the most recurring issues when it comes to their compliance?

Bello Endurance

safe guiding is one of the biggest issues I want to talk about. You know, it's it's it's a very, very big issue here, you know. And you also talk about training and operations in terms of the leadership, you know. So some people just get into the system, really do not understand the terrain which they are. It's just like, you know, I I have the money, I want to invest in the head in the healthcare sector. But you do not know how to run it. You don't know what components of things you need to do, what could lead to safeguarding, what is expected of you. And so these are big issues. And so what we have done is to ensure that we have set up a recurring training for these leaders of organization to ensure that they are well trained and understand how to navigate when there is a problem. What can lead to safeguarding and how will they be able to manage that situation? that safeguarding is adequately managed, you know, safeguarding is is properly is handled when it happens how with the staff if it happens how would they be able to navigate through that system what would they learn it's called for example it's called lesson learned if there is a safeguarding how will you be able to pass that safeguarding to people so that they will understand so that such does not happen again you know so that we've really helped many organizations to face Many organizations, for example, that you've seen that perhaps are are in trouble with CQC, it could be as a result of safeguarding that they don't understand that concept, you know. And another problem again is efficiency in running when you don't have competent staff. And that could be as a result that you do not train them, you know. And so we fish out these things, make this report to the directors, they look at it, run with it, and then implement it. It's one thing for us to fish out the problem. It's another thing for you to implement it. But one thing I can guarantee anybody is that once you implement the report which we have given you, there is every certainty that the organization will move in the right direction.

The First Thing He Checks At A Care Provider

Mikulas Zach

you mentioned CQC earlier on. if a care provider comes to you, what's kind of the first thing that you look at?

Bello Endurance

Well, it it depends on what where the problem, I mean, what he or she what he she really wants to achieve. You know, some care providers know that they need a turnaround in the system, perhaps maybe they've been rated very poorly and they need a turnaround. And some are already there, but what they want is please come and help us see the system so that we can also we can ensure that we are always in compliance, right? And so, what I want to look at is efficiency in running the system first off. If you are a pro-care provider, you come to me. I want to test whether you are quite very efficient. I want to test whether you are following the right standards. There are four core things I really want to test. The four core things. Are they caring? Are they is there safety in the system? Do they really understand what they're doing? And then are they efficient in running the system? And who are their managerial team? Are they sound? No, if I test this four, five quickly, I will know whether there's a problem in the system. So by the time I test each of these, I will figure out where the problem is. And then we can now set up to do a compliance audit and then do what they call a risk assessment to pinpoint the areas. Then we audit those parts and then generate the reports that the directors could use.

Advice For Providers Behind On Compliance

Mikulas Zach

tell me Bello, we talked about mistakes. what sort of advice could you give if there is a you know a care provider that's listening right now and might know that you know he's behind compliance? what would be kind of your your main levers or tips that you could give them?

Bello Endurance

Hmm. I mean, if mistakes have happened, if your system, your care homes, your hospitals, you feel that you are not running up to efficiency, I think it's that look, there is nothing like reach out for people who are very knowledgeable, and that is why we are here at Ebera Consulting. We would rather want you to come to us so that we can help you fix this problem. If you are not one thing is for you to open is another thing is for you to run it efficiently. If you understand you have a problem, you better open up. It's it's better for you to open up than to close a problem, right? And so it's for you to consult with people who are far ahead of you, who knows the regulations and who knows how to face where there is a problem. If you are sick, for example, you will be advised to see the doctor and then the doctor make recommendations for you as to take this medication, take that medications. I would advise anybody if there is a problem in your in your in your organization, reach out to experts that could help you dissect what the problem is, make proper recommendations as how as to how to face it. And that will be the recommendation I will make for for them. at the very consulting, we are we open hands to people who feel that we they will need our expertise to help them check how they are really running their system. You know, it is not for you to say yeah my system is being run adequately because There will be that familiarity and there will be that self-confidence that you're doing the right thing. But rather it will be better for you to say, I know we are doing quite well. I will need a third eye to help us see whether are we really doing the right thing? Because the third eye will not be biased, right? The third eye is we want to like really open up every component of the operations as to how they are really performing. Are they performing to the optimal best as to what's CQC or the regulators we want to see and that is where a barrier for example will be coming to help you see each of those components as to are they really up there. You know the goal is to make sure that you are safe in terms of operation and that is what we offer.

How AI Is Changing Healthcare Compliance

Mikulas Zach

Hmm. tell me Belle in terms of you know AI as it's a big topic I think in all industries, what do you see and how do you see it play out in healthcare compliance? How do you use it?

Bello Endurance

Well, you can't take AI away from the health and social care sector. That is is a very big one that would that we change the way things are really going now. look, we just cannot keep talking about many organizations adapting to it, you know. so for AI, for example, In as much as we want to say that organizations should not adopt it immediately, but what it does it help you streamline your activities in a very organized and efficient way. Right? So I I would want to say in a very in just a few years away from now, we take virtually every aspect of the head and share here. But what will happen is that operators will need to retrain themselves to use the tools. AI will not take over everybody, but what you need to do as an operator is train yourself that when you are implementing AI in the system, you know how to use it. Because just look at it. Everything we do now is AI, right? Even this podcast you're doing, there's a way AI is going to assist you in a whole lot of ways. And the AI also in healthcare, it's it's really taking taking a lot of shape. Many people now use AI in terms of documentation, in terms of also checking the risk. In terms of doing a whole lot of writing that before you have 10 people, five people doing, but just with AI, you can do your documentation, you can check your risk assessment, you can do care planning, and you can do care planning for 20, 30 people. Before now, you have to separate doing care planning with five, six human beings. But just with a single button, AI will do that for you. But what the problem will be is that if you do not train yourself to use that tool. You know, is training yourself to make sure that you are very efficient in using tools that that are mean AI that is efficient in the health and social care sector. And that is the advice I will give anybody.

What AI Will Never Replace

Mikulas Zach

No, I think you know it's spot on. I wonder what is the thing that you think AI won't replace? Because I think there is a lot of stuff going on, you know, on LinkedIn and everywhere where people are replacing sales, marketing. But what do you think in your opinion AI won't ever replace?

Bello Endurance

well I I I just I just can't that's a very big question. A very, very big question you asked me what AI will not replace. look, let me tell you something about AI. AI is a thinking of human, right? Put together and then you allow it to continue to grow. It's a network of ideas. So I I just cannot tell you what AI cannot replace. But the only thing I know AI cannot replace is your human effort. The only thing I feel that AI cannot replace is human effort. So, what do I mean when I say human effort? You went to school to study, for example, accounting, right? Now, AI has taken over accounting. AI can prepare accounting better than you and I. It knows the debit, it knows the credit, it knows the balance sheet, it knows the PL, right? Now, but one thing AI cannot replace is that ability for you to say, you know what, I'm not going to stop learning. I want to continue to learn. They've told you that AI is doing this. So, as a doctor, what you should do or as a nurse or as a healthcare professional is okay, what can I learn to upskill myself? Now, what AI will not replace for you if you this if you decide to say, you know what, I don't want to upskill myself, and then you remain where you are. And so it's it's it's only logical that you look at AI as something that's even more than educational, you need to learn it. And know how to implement it. So if you want to ask me what AI cannot replace, I will tell you it is you that AI cannot replace. And you have an advantage, either to obscure yourself or to remain stagnant. If you remain stagnant, it becomes a very huge consequence because people who does risk reskill themselves will be far better and will be well placed and positioned.

Mikulas Zach

Yeah, I I think I definitely agree with you there. One of the main things that I think won't ever be replaced, at least for the time being, when humans are running around you know the world, is relationships, especially the business ones, because you know it's still business to business, but at the same time there are two people you know on each side. That are making the decisions. hopefully, right? Hopefully you don't have AI do the decisions. tell me, one of the things that I find really interesting, and I wonder how have you cracked it, if you have cracked it, is marketing for you know the type of work that you do because compliance, it's it's very a lot of people feel like it's almost like a checklist item, same with accounting. so how do you currently do marketing? Do you have anything working or is it you doing great work and then people referring you to others?

Marketing A Compliance Firm

Bello Endurance

Well, referral is part of the marketing though, in fairness. And we we write a lot of I write a lot of articles. I write a lot of articles on major posts like LinkedIn, like on my WordPress, Instagram, TikTok, we do a lot of articles that relate to the content or the the brand or what we do. Right. From those writing, people reach out to us, you know, the reach out and then from there we start the conversation. I also do advert on a goggle sometime. I do a lot of advert on goggle. And healthcare providers who need me perhaps will reach me from there. And those that reach me, like people whom I've worked for make referral. And so the referral does move on and go and go and just just like that. That is how we Dual marketing.

The Vision For Elberra

Mikulas Zach

So tell me what's kind of next for you know Albera consulting, what's the vision for the firm?

Bello Endurance

Yeah, the the the vision for the firm is to move from the way the f from the current phase it is. The face the first phase is to provide healthcare compliance. The second phase will move into the finance part of it. you know, the healthcare fin the telemarketing, telemedication out of it, you know, digital marketing that of it, which we are already constructing so that everybody can have as I mean, Everybody can have can use one of our digital platforms to have a compliance in their hospital or whatever it is. Ebera will also move very fast into other core areas such as finance, capital market, capital raising, which is part of what we do, but we've not actually brought that to the market. And I also see a barra moving from just moving away from UK and going to other European countries, Asian countries and America.
